Becoming a Member at SD Medical Federal Credit Union has never been easier. Simply follow the steps outlined below and use the forms provided. Enter your information into each of the fields and then print the forms. You can even save a copy on your computer and make changes as needed. Step 1

If you haven't already done so, fill out our Membership Application and select the products and services you would like to apply for. Then bring it to one of our convenient branch locations. (You can also mail it to our main branch.)

Step 2

Once you have completed your Membership application and have been assigned an Account/Member Number, open our Direct Deposit Form, enter your account information and print a copy of the form. Bring this to your company's Payroll or Human Resources Department to begin your Direct Deposit with SD Medical. (Your payroll department may require that you fill out their form as well.) * Please note that Federal Employees and Active Military Personnel should use the Federal Direct Deposit Form.

If you would rather have only a portion of your paycheck deposited into your SD Medical Account, use our Payroll Deduction Form. (Please note that some employers do not offer this service.)

Step 3

Fill out a copy of our Request to Change Automatic Payments Form for each company that automatically deducts payments from your old account. (such as Internet Service Providers, Gyms, Insurance Companies, etc.)

Step 4

Once all of your Automatic Payments have been transferred to your new SD Medical account, fill out a copy of our Request to Close Account Form and send it to your old Financial Institution.
